To use the Change of Address form or the Patient Registration Form just click the image below. Then click open. Print the form, fill it out, then bring it with you on your next visit. Having this form filled out and ready to hand to the receptionist will decrease the amount of time you spend in our waiting room. For now, you must have Microsoft Word or Microsoft Works to use this feature. In the future we will make the form available in other formats to make it easier to use.
